Preventing Hypoxemia During Pulmonary Sleeve Resections

To the Editor:
Dr. Ng reports the effectiveness of jet ventilation during a sleeve resection and one-lung ventilation to prevent desaturation (1). Although this may be an effective method, jet ventilation is not always immediately available and may not result in a quiet surgical field.
Another option in this situation is to ask the surgeon to place a vascular clamp on the main pulmonary artery in the surgical field. This will reduce the shunt flow and immediately increase the oxygen saturation.
