Preemptive Analgesia by Nonsteroidal Antiinflammatory Drugs

Gramke et al. (1) demonstrated preemptive analgesia from sublingually administered piroxicam, a nonsteroidal antiinflammatory drug (NSAID). In their discussion the authors referred to a previous investigation by OHanlon et al. (2) that also demonstrated preemptive analgesia. Other NSAIDs, including IV ketorolac (3) and flurbiprofen (4,5) have also been reported to provide preemptive analgesia. In our study (5) we confirmed that preemptive and long-lasting analgesia from flurbiprofen was independent of residual drug concentration (5).
The characteristics of preemptive analgesia have been reviewed by Kelly et al. (6). Available data suggest anesthesiologists should apply multimodal analgesic techniques, including NSAIDS, to achieve the benefits of preemptive analgesia.
