Acute Hemodynamic Collapse After Induction of General Anesthesia for Emergent Pulmonary Embolectomy

Rosenberger et al. (1) reported their institutional experience of hemodynamic deterioration after induction of anesthesia for emergent pulmonary embolectomy. We believe that stronger consideration should be given to using femoro-femoral cardiopulmonary bypass prior to inducing anesthesia in patients with severe right ventricular dysfunction. The femoral vessels can be readily identified and cannulated under local anesthesia, and the procedure is well tolerated (2). The small risk of venous thromboembolism associated with instrumentation of the femoral vein in the setting of pulmonary embolism (3) has to be weighed against the substantial risk (19% in the series reported) of hemodynamic collapse on induction.
Gibbons animal experimentation led to the development of cardiopulmonary bypass (4). Ironically, the impetus was a patient who died during emergent pulmonary embolectomy at MA General Hospital.
