Paravertebral somatic nerve block (PVB) provides improved analgesiaand decreased side effects compared with general anesthesiafor breast surgery. The analgesia is limited with single injectionPVB to the duration of the local anesthetic. Continuous paravertebralcatheters and disposable home infusion systems allow extendedanalgesia after major breast surgery while maintaining the ambulatorystatus of the patient. We describe the successful use of continuousparavertebral anesthesia in two patients undergoing major breastsurgery. A novel needle system for paravertebral catheter insertionis also presented.
